I side-sleep in a CPAP mask

A mask frame sits proud of the face and does not compress. On a surface that has already bottomed out, the frame is levered against your cheek — the same mechanism as an earbud, moved to the face, with a larger and more awkwardly shaped object doing the levering. Correct loft matters more here than anywhere else in this index, because a head that is tilting is a head whose mask seal is being asked to stretch.

Two problems at once, and only one of them is the pillow's. The mask frame is a rigid object pressed between your face and the surface, and the hose has to go somewhere that is not under your shoulder. A pillow can address the first and get out of the way of the second. It cannot address anything about the therapy itself.
